No, I have only observed them (once) from afar, so I have never been bitten by one, and hope never to experience this. To be fair, they are not within my line of work.
No. I think they are mostly freshwater fish, so I haven’t encountered them. But, my husband did spend 6 months working in the rain forest in Peru and still insists that their reputation is overly harsh and they are really mild mannered and he has not been bitten despite swimming in rivers with pirhana.

Not all pirhanas are meat eaters, some are vegetarian, have human-looking teeth and eat a plant-based diet… none have eaten me…yet…
